Breaking Down Silos: The Key to Effective Collaboration and Safety Signal Management

Insights from an XTalks webinar on collaboration between Medical Information, Pharmacovigilance, and Regulatory Affairs

In pharmaceutical organizations, critical safety information is often received by multiple teams. Medical Information (MI) receives inquiries from healthcare professionals (HCPs) and patients, Medical Science Liaisons (MSLs) hear concerns from healthcare professionals, Patient Support Programs collect patient feedback, and Pharmacovigilance (PV) monitors adverse events.

Each of these teams is doing its job, but what about the bigger picture? And how are the dots connected, if at all?

When Critical Safety Information Falls Through the Cracks

The webinar opened with several examples that illustrated how communication breakdowns can occur across organization functions:

  • The Missed Signal: Medical Information receives inquiries about an unusual symptom. MSLs hear similar feedback from HCPs. Neither reaches the threshold for escalation on its own. The pattern goes unrecognized until a regulator potentially identifies it.
  • The Communication Disconnect: Pharmacovigilance identifies a safety signal and works with Regulatory to update the label. A Dear Healthcare Professional letter is issued, but MI and Medical Affairs (MA) aren’t brought in early enough. They're suddenly fielding questions without updated responses or approved messaging.
  • The Regional Disconnect: Global Regulatory issues a safety communication, but local teams aren't prepared with market-specific context or alternatives.

In each case, individual functions fulfilled their responsibilities, but insufficient cross-functional communication created unnecessary risk for compliance, trust, and ultimately patient care.

One of the panelists shared the JAK inhibitor experience from 2021. When FDA issued black box warnings based on long-term safety data, PV and Regulatory submitted the required documentation. However, MI and MA weren't prepared for the flood of questions from HCPs and patients, and MSLs lacked the training and FAQs needed to respond consistently.

The example highlights an important reality: even when critical information existed within the organization, it hadn't reached everyone who needed it when they needed it.

How Cross-Functional Collaboration Strengthens Safety Signal Management

The panel shared several practical ways organizations can improve collaboration and strengthen safety signal management across functions.

Bidirectional Communication

PV shouldn't only receive insights from MA and MI. It should also ask these customer-facing teams to verify emerging signals, and let communication flow both ways.

Shared Visibility

All signal sources, including MI inquiries, MSL insights, patient support program feedback, and adverse event reports, should feed into a unified view. One panelist emphasized the importance of a shared tracker or dashboard so every function has visibility into emerging issues and no team is caught unprepared.

Cross-Functional Forums

Regular signal review meetings should include PV, Regulatory, MA, MI, and other relevant functions. These meetings should establish clear checkpoints: How was the signal detected? Has it been validated? What actions need to happen next? Who is responsible?

Including Medical Information and Medical Affairs as early as possible is critical because they need time to prepare responses, briefing materials, and field guidance before inquiries begin.

The panel also recognized that organizations operate differently. Large pharmaceutical companies may have mature governance structures, while smaller biotech organizations often rely on leaner teams with individuals wearing multiple hats. Regardless of size, the panel recommended three foundational practices:

  • Single inbox: Route all safety-related inquiries through one centralized channel.
  • Shared tracker: Give all relevant functions visibility into signals under evaluation.
  • Regular connects: Hold recurring check-ins, even informal ones, so teams can compare observations and coordinate next steps.

Where AI Fits into the Process

No discussion about modern pharmacovigilance would be complete without addressing AI. The panel agreed that AI can support safety signal management by eliminating duplicate effort, automating literature searches, and reducing administrative burden, allowing experts to spend less time finding information and more time analyzing it.

However, they were equally clear that AI should augment human expertise, not replace it. Signal validation, causality assessment, and benefit-risk evaluation remain responsibilities that require scientific judgment and cross-functional collaboration.

Keeping Patients at the Center

One of the panelists closed the discussion with a simple but powerful reminder:

"Remember why we are here. We want patients to get better, to have a better quality of life. Working together as a team rather than in silos is crucial to keeping the patient at the center."

Ultimately, the science can be right and the processes compliant, but if critical information doesn't move across functions at the right time, patients and healthcare professionals are left without the answers they need.

This webinar wasn't simply about improving operations. It was about strengthening collaboration to support better, safer outcomes for patients.
